Konnected Technologies Inc. (KTI) revolutionized social health measurement with our patented Location Monitoring and Evaluation (LME) system when we launched in 2022. Our Intelligence-as-a-Service platform transforms community data into actionable intelligence through our proprietary database architecture and API connections. Providers gain access to our comprehensive analytics dashboard, secure data exchange protocols, and customizable alert thresholds that integrate seamlessly with existing EHR systems. Founded in Colorado by Brittany Katalenas, Kooper Kuehl, and Mark Milburn, with clinical innovations from Dr. Bianca Brandon (LCSW), KTI began serving underserved communities but now operates in 13 states across two continents. Our implementations demonstrate proven ROI, reducing emergency service utilization by 35% and improving preventative care engagement by 48%, while helping partners secure over $67 million in funding. From rural community clinics to large urban hospital systems, our scalable technology adapts to diverse provider environments, making us the ideal innovation partner for Colorado's healthcare future.

Link Health is a nonprofit initiative using AI-powered tools and a team of Certified Patient Navigators to bridge healthcare and social support access for Medicaid enrollees. By embedding certified patient navigators in clinical settings, Link Health turns healthcare encounters into enrollment opportunities for cash assistance programs like SNAP, WIC, Lifeline, LIHEAP, and TANF. Designed with health equity in mind, the platform uses generative-AI and natural language processing to screen patients, autofill forms using OCR technology, and enable real-time, multilingual enrollment in cash assistance programs. The solution blends human support, chatbot, and SMS workflows to engage low-income, BIPOC, and non-English-speaking patients to complete critical benefit applications successfully. Founded by ER physician Dr. Alister Martin, Link Health has helped over 3,300 patients unlock more than $4.3 million in benefits. Having demonstrated impact in Massachusetts and Texas, Link Health is preparing to launch in Colorado to advance health equity for medically underserved communities.

RenewRx is a maternal health platform transforming prenatal care between OB visits—with minimal to no cost for patients. Focused on high-risk pregnancies like gestational diabetes (GDM), which disproportionately impacts Medicaid and safety net populations, RenewRx delivers remote patient monitoring and behavior-based nutrition and lifestyle coaching tailored to each patient’s unique social determinants of health. In our 2024 pilot, 100% of participants reached glucose control within 4 weeks, NICU admissions dropped significantly, and 95% reported reduced stress and improved care. By addressing barriers like food access, health literacy, and daily overwhelm, our coaches help patients build sustainable habits that drive better outcomes. The platform integrates seamlessly into provider workflows and enables clinics to bill existing RPM and care management codes—generating new revenue without increasing staff burden. Proven with commercial payers, RenewRx is now expanding to support Medicaid and safety net communities across Colorado.

Joyuus empowers postpartum women to manage their postpartum self-care while providing a clear path to seeking care through risk assessments and resources. Our predictive model (under development with HPE) intends to create the pathway to early identification for mental health conditions like depression and anxiety; physical health conditions like hypertension, cardiac conditions, or hemorrhage; and a link between mental and physical health and SDOH. The goal is to ensure maternal health care is truly integrated, preventative, and inclusive. The Joyuus postpartum platform will ultimately broaden beyond postpartum to become a springboard for broader women’s health data and insights.
