Cara AI is a multimodal AI platform built to standardize in-home health assessments so Medicaid members age in place, avoid the ER, and access the benefits supporting their wellness. Cara guides care coordinators through state-mandated LTSS assessments and completes roughly 70 percent of the visit remotely through SMS and weblink workflows before the coordinator arrives, subject to plan workflow preferences . The platform captures home safety risk through computer vision, scans medication bottles with OCR, conducts voice and chat health risk assessments, screens for food insecurity and social needs, and returns coded findings to the plan in real time. Founded in 2025 by Dr. Renee Dua, a practicing nephrologist and repeat health tech founder, Cara is deployed with more than 200 assessments completed and is running pilots across Medicaid LTSS, Medicare Advantage, and post-acute discharge


ChartR Health builds AI-powered tools that help healthcare teams turn complex clinical and operational data into clear, real-time decisions. The platform analyzes patient records, documentation, and workflows to identify risks, highlight gaps in care, and surface patients who require follow-up, allowing teams to act earlier and more consistently. By integrating directly into existing clinical and operational systems, ChartR helps providers move beyond delayed reporting and manual chart review and respond to issues as they emerge. The platform is designed to support care teams working in high-volume, resource-constrained environments, where timely access to information is critical. ChartR supports health systems and clinics in improving care coordination, reducing inefficiencies, and delivering more consistent, data-driven care without adding complexity to day-to-day workflows.

Elythea is the voice AI engagement layer for safety-net Medicaid plans. Our multilingual agents complete the clinical conversations overworked call centers cannot: Health Risk Assessments, welcome calls, postpartum follow-up, redetermination outreach, and warm transfers to care managers. Across 8 US states, Elythea has served a diversity of patients from rural indigenous BIPOC mothers served by resoure-constrained CBOs/nonprofits/last-mile safety-net provider systems to members of of a Fortune 25 health plan. One recent partnership lifted HRA completions 160 percent in five days. Elythea is cash-flow positive in year four, goes live in under nine days, and built for the populations safety-net plans must reach.


Two years ago we started deploying inside a licensed marriage and family therapy practice. Today we have documented evidence that our platform deflects psychiatric emergency visits for acute behavioral health distress before they happen and that symptom tracking between visits helps improve outcomes. Jeeva builds the technology layer that lets rural primary care physicians act on behavioral health needs they see every day but currently have no infrastructure to address — connecting patients to psychiatry expertise through continuous mobile monitoring, AI-assisted clinical summaries, and automated referral coordination. Jeeva's active deployments are in California's CYBHI program in San Bernardino and Riverside (80+ Medi-Cal youth, 2+ years, ER deflection documented) and the University of Florida's federally-recognized PMHCA hub in rural Florida (14–20 rural PCPs, BAA executed). Mobile-first, caregiver-inclusive and soon to be bilingual. Some communities we serve don't walk into a therapist's office but Jeeva enables PCP's trusted platform reach them..

The company combines clinical whole genome sequencing, multilingual genetic counseling, and a mobile-friendly care navigation platform that helps families move from uncertainty to clear next steps faster. Supported in part by funding from Colorado’s Office of Economic Development and International Trade (OEDIT), Jura Health is preparing to launch its next-generation patient platform in Q4 2026 to connect families with specialists, therapies, diagnosis-specific resources, and social support services. Jura Health is especially focused on Medicaid and underserved communities, where delays in diagnosis and barriers to care are most common. By integrating genomics, digital health, and community partnerships, Jura Health is building a more equitable future for children’s health in Colorado and beyond.


Lexi is a HIPAA-compliant, real-time voice AI platform designed to improve language access for the 30 million U.S. patients facing language barriers in healthcare. Medically fine-tuned and built for clinical workflows, Lexi supports multilingual communication across the full care journey, from scheduling and check-in to consultation, discharge, and follow-up, through phones, tablets, and existing healthcare infrastructure. Built by a multilingual team from Harvard and MIT, Lexi supports both common and underserved languages, including the industry’s first Haitian Creole voice AI interpreter. Backed by a $1.4M pre-seed round, Lexi is already live with paying customers, a growing pipeline of 20+ Federally Qualified Health Centers, and a Mass General Brigham Emergency Department research pilot launching in 2026.

Mann Method is a women-owned, for-profit healthcare organization delivering specialized rehabilitation services for individuals with Down syndrome and related intellectual and developmental disabilities (IDD). Our model integrates evidence-informed physical therapy (PT), occupational therapy (OT), caregiver training systems, and longitudinal functional outcome tracking. Replacing traditional, fragmented episodic care, we deliver a continuous, lifespan-oriented functional rehabilitation model. DDAI runs on the ultrasound equipment a Colorado FQHC already owns, requires no new hardware, and is operated by the sonographer in the routine prenatal workflow — making specialist-level dating accessible to safety-net patients who do not have on-site MFM access.
