Video and Creative Works Waiver and Release

I hereby irrevocably agree and consent that Colorado Digital Health d/b/a Prime Health (“Prime Health”), or a party acting on Prime Health’s behalf, may capture photographs, video content, and audio content which includes my name, image, likeness, voice, actions, or other identifying characteristics (collectively, “Content”) for the “Innovation Academy” series.  I have all necessary right, permission and authorization from my employer or other third party to provide the Content to Prime Health and to execute this Waiver and Release.  Prime Health has the irrevocable right, which I hereby grant, to post or repost any Content to any media channels, which Content may remain there in perpetuity without any obligation of Prime Health to remove or effect removal of such posts. I will have no right to monetary or material compensation or royalty for use of Content by Prime Health, and I hereby waive any right or opportunity to inspect or approve any Content.
